8.9 million deaths a year: the warning signs before a heart attack

The symptoms documented in the study cover a wide range: chest pain, chest heaviness, chest burning, palpitations, fatigue, sleep disturbance, shortness of breath, dizziness, anxiety, sudden heat or cold, back pain, and vomiting. Their variety is part of what makes them easy to dismiss as unrelated or minor complaints.

The breadth of this list underlines why cardiovascular awareness matters. With ischaemic heart disease ranking as the world’s top killer, recognizing these signals early is one of the few practical levers available before an acute event occurs.

What are prodromal symptoms?

The term ‘prodromal’ refers to early symptoms that precede the full onset of a disease or acute event. In the context of heart attacks, research published in The National Library of Medicine identifies these signals as appearing days or even months in advance. Despite their clinical relevance, the same research notes they are frequently dismissed by both patients and the healthcare professionals who first assess them.

Fatigue, breathlessness and weakness: when tiredness signals something more serious

Fatigue is one of the most commonly underestimated warning signs. Extreme tiredness or a sudden inability to perform routine tasks can be linked to reduced blood flow to the heart, typically caused by narrowed arteries. Because fatigue is so common in daily life, its connection to cardiac health is frequently ignored.

Shortness of breath occurs when the lungs do not receive adequate oxygen for the body to function properly. WebMD notes that “shortness of breath and feeling tired can be signs of heart failure” — a condition defined as the heart’s inability to pump blood around the body effectively.

Sudden, unexplained weakness is another signal worth taking seriously. While its causes can be varied, an abrupt drop in physical capacity without an obvious reason may indicate the body is under cardiovascular stress.
