The nine documented warning signs are: sudden weakness or numbness on one side of the face, arm, or leg; sudden difficulty speaking or understanding speech; sudden drooping on one side of the face; sudden trouble walking or maintaining balance; a sudden, severe headache unlike any previous headache; sudden changes in vision including blurred, double, or lost vision; sudden confusion or inability to understand what is happening; sudden difficulty swallowing; and sudden loss of coordination or clumsiness.
Stroke symptoms can also be subtle. The source notes that the only visible sign may be a slightly slurred sentence, a weak hand, a sudden inability to read, or a brief episode of confusion. A person may even insist that nothing is wrong — that does not mean the situation is harmless.
Critically, a stroke does not have to produce every classic symptom at once. One sudden neurological change may be enough to justify emergency evaluation. Sudden trouble speaking alone can be a stroke. So can sudden vision loss, or sudden weakness on one side.
Two types of stroke, one emergency response
An ischemic stroke occurs when a blood clot or obstruction blocks blood flow to part of the brain — the most common form. A hemorrhagic stroke occurs when a blood vessel ruptures, causing bleeding in or around the brain. Both damage brain tissue, and both require the same immediate response: call emergency services without delay. The distinction matters for treatment — which is why brain imaging is typically required before certain therapies are administered.
FAST: the four-letter method that saves lives
Medical guidance has long promoted a simple mnemonic to help bystanders act quickly. FAST stands for Face, Arms, Speech, and Time.
F — Face: Ask the person to smile. Does one side droop? A — Arms: Ask them to raise both arms. Does one arm drift downward or feel weak? S — Speech: Ask them to repeat a simple sentence. Is their speech slurred, confused, or unusual? T — Time: If any of these signs are present, call emergency medical services immediately.
